3. Auto Accident & PIP (Personal Injury Protection) Notice – Florida Residents

If you are seeking care following a motor vehicle accident in Florida, please be aware of the following:

• Florida’s 14-day rule applies: Patients must seek care within 14 days of the date of accident to access PIP benefits.
• Services provided for accident-related injuries may be billed to PIP, third-party insurance, or on a lien basis when applicable.
• We work with attorneys and may submit documentation on your behalf but do not guarantee payment or legal outcomes. You remain responsible for any uncovered services or denials.

4. Financial Responsibility

By receiving services from our practice, you agree to the following:

• You are financially responsible for services rendered, whether covered by insurance or not.
• Estimates provided are not a guarantee of insurance reimbursement.
• Non-covered services, such as IDD Therapy, red-light therapy, and maintenance chiropractic care, are payable at the time of service.
• A 72-hour cancellation/refund policy applies to all prepaid discounted services per Florida law (F.S. §456.062).

5. HIPAA & Patient Privacy

We are committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
